This study aimed to understand the meanings and significance of organ donation for family members who experienced the process of Brain Death. It is a qualitative, descriptive, and exploratory research using semi-structured interviews with ten potential donor family members, analyzed with IRaMuTeQ software. Results highlighted that clear and empathetic communication by the multidisciplinary team is central for understanding the process and accepting the brain death diagnosis, directly influencing the decision to donate. Five main dimensions were identified regarding the family experience: the importance of communication, the process of accepting brain death, the technical understanding of the diagnosis, the dilemma between grief and altruism in donation, and the social and family dimension of the act of donating. Organ donation emerged as a possibility for reframing grief and achieving a dignified death, transforming the pain of loss into an act of solidarity. This study contributes to improving family approach practices and highlights the need for psychological support and qualified communication, which are essential to strengthen public organ donation policies in Brazil.
